The terms that apply when you browse, order, and purchase from Form Precision Labs.
1. Parties & scope
These Terms of Sale govern the purchase of research-grade peptides and laboratory reference materials from Form Labs LLC d/b/a Form Precision Labs ("the Company"). By accessing the order process or placing an order, you agree to these terms and the linked Research-Use Policy, Shipping Policy, Refund Policy, and Privacy Policy.
2. Research-use representation
You represent and warrant that you are at least 21 years old, are a qualified research customer purchasing exclusively for in-vitro research, analytical method development, identity verification, or laboratory evaluation, and are authorized by the identified organization or institution to do so.
3. Prohibited use
You agree that materials are not for human or animal consumption and that you will not use, or permit the use of, any material for therapeutic, clinical, diagnostic, dietary-supplement, dosing, injection, ingestion, or administration purposes.
4. No resale for human or animal use
You agree not to resell, transfer, or distribute any material for human or animal consumption or administration, or in any manner inconsistent with these terms.
5. Right to cancel non-compliant orders
The Company may refuse, suspend, or cancel any order, in whole or in part, that it reasonably believes is intended for a prohibited use, involves a restricted product, or violates these terms or applicable law.
6. Acknowledgment at checkout
Checkout requires separate affirmative attestations covering age, qualified research status, no human or veterinary consumption, and no administration or prohibited resale. The Company records the attestation text version, acceptance timestamp, researcher type, account, and order with the submitted order.
7. Regulatory status & buyer responsibilities
Materials are not drugs, dietary supplements, cosmetics, foods, or medical devices and are not approved by the U.S. Food and Drug Administration to diagnose, treat, cure, or prevent disease. The buyer is responsible for determining whether purchase, possession, handling, storage, transfer, and use are lawful and appropriate in the buyer's jurisdiction and facility and for obtaining and following current supplier safety documentation.
8. Product information, inspection & documentation
Product-page identity data and Certificates of Analysis describe the referenced lot or component lot. Composite products may require separate component and finished-product documentation. Buyers must inspect labels and lot documents before use and promptly report discrepancies. A product page or CoA is not a substitute for a supplier-issued Safety Data Sheet.
9. Disclaimer & limitation of liability
Materials are provided "as is" and solely for research use. To the maximum extent permitted by law, the Company disclaims implied warranties and is not liable for damages arising from improper handling, storage, transfer, prohibited use, or misuse. Nothing in these terms excludes liability that cannot lawfully be excluded.
10. Governing law & venue
These terms and each order are governed by the laws of the State of Wyoming, without regard to conflict-of-law rules, except that the Federal Arbitration Act governs the interpretation and enforcement of the arbitration agreement below. For any dispute that is not subject to arbitration, the parties consent to exclusive jurisdiction and venue in the state or federal courts serving Sheridan County, Wyoming.
11. Individual arbitration & class-action waiver
Before filing a claim, either party must send written notice describing the dispute and requested relief and allow 30 days for an informal resolution. Except for an eligible individual small-claims action or a request for temporary injunctive relief, any dispute arising from these terms, the storefront, or an order will be resolved by binding arbitration on an individual basis in Sheridan County, Wyoming. Arbitration will be administered by the American Arbitration Association under the rules applicable to the dispute. Each party waives trial by jury and agrees that claims may be brought only in that party's individual capacity, not as a plaintiff or class member in any class, collective, consolidated, representative, or private-attorney-general proceeding. If applicable law makes a portion of this paragraph unenforceable, that portion will be severed and the remainder will continue to apply to the fullest lawful extent.
12. Severability & changes
If a provision is found unenforceable, the remaining provisions remain in effect to the extent permitted by law. The version presented and affirmatively accepted at checkout governs that order. Material updates apply prospectively unless law requires otherwise.
