How Form collects, uses, retains, and shares personal information.
This policy explains how Form Labs LLC d/b/a Form Precision Labs ("Form Precision Labs," "we," "us," or "our") collects, uses, retains, and discloses personal information through formlabsusa.com, customer accounts, research-order checkout, support, and related business operations.
1. Information we collect
Depending on how you interact with us, we may collect the following categories of personal information:
- Identifiers and contact information: name, email address, telephone number, account identifier, shipping address, IP address, and device identifiers.
- Qualified-buyer information: the researcher type selected for qualified-buyer review.
- Commercial information: products considered or ordered, order history, referral or discount code, payment method, payment-confirmation reference, shipping status, returns, and support history.
- Internet or electronic activity: pages viewed, referring page, browser and device information, interactions with the site, and analytics events.
- Compliance records: the content, version, date, and time of research-use, age, and prohibited-use attestations, together with the researcher type selected.
- Communications: emails, support requests, and other information you choose to provide.
We do not ask you to submit a Social Security number, employer identification number, bank-account password, payment-card number, or medical information through the storefront form. Optional account authentication is handled by our authentication provider, and payment details are entered only in NoRamp's secure hosted checkout.
2. Sources of information
We collect information directly from you; automatically from your browser or device; from authentication, hosting, analytics, communications, shipping, and order-management service providers; and from referral partners when you use a referral link or code.
3. How we use information
We use personal information to operate and secure the site; create and authenticate accounts; verify qualified-research eligibility; process, review, fulfill, and support orders; document required attestations; detect fraud or prohibited use; provide lot and shipping information; honor legal requests; maintain business and tax records; analyze site performance; and communicate about our services.
4. How we disclose information
We may disclose the categories above to vendors that support authentication, hosting, analytics, communications, order management, payment confirmation, fraud prevention, shipping, professional advice, and data storage. We may also disclose information when reasonably necessary to comply with law, protect rights or safety, investigate misuse, or complete a merger, financing, acquisition, or sale of assets.
We do not sell personal information for money. If advertising or analytics technology is enabled, certain device or activity information may be considered "sharing" for cross-context behavioral advertising under California law. You may opt out by emailing us with the subject "Do Not Sell or Share". We also process browser-based opt-out preference signals, including Global Privacy Control, where legally required.
5. Retention
We retain each category only as long as reasonably necessary for the purpose for which it was collected, including fulfillment and support, qualified-buyer and prohibited-use controls, legal and tax obligations, dispute resolution, fraud prevention, and security. Retention varies by record type. We delete or de-identify information when it is no longer reasonably necessary, unless law requires or permits longer retention.
6. Cookies and local storage
The site uses cookies, session storage, and local storage for account sessions, cart contents, shipping-form convenience, research-access acknowledgments, referral attribution, security, and analytics. Blocking these technologies may prevent account, cart, or checkout features from working correctly. Where required, a recognized opt-out preference signal is treated as a request to opt out for that browser or device.
7. California privacy rights
If the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA), applies to us and to your information, a California resident may have the right to request access to categories or specific pieces of personal information; information about sources, purposes, and recipients; deletion; correction; opt-out of sale or sharing; limitation of certain uses of sensitive personal information; and non-discriminatory treatment for exercising a privacy right.
Submit a request by emailing formlabsllc@gmail.com with the subject "Privacy Rights Request" or calling (813) 561-2953. Describe the right you want to exercise and the email address associated with your account or order. We will use information already maintained by us to verify the request to the degree required by law. We may ask an authorized agent for proof of authority and may verify your identity directly. You do not need to create an account or accept these Terms to submit a request.
8. Data security
We use administrative, technical, and organizational safeguards designed to protect personal information. No transmission or storage system is guaranteed to be completely secure. Please do not send sensitive financial, authentication, or medical information by email.
9. Age restriction
The storefront is intended only for adults age 21 or older acting for qualified research purposes. We do not knowingly collect personal information from children. If you believe a child submitted personal information, contact us so we can review and delete it as appropriate.
10. External services
Our site may link to or use services operated by other companies, including authentication, analytics, financial, and shipping providers. Their privacy practices are governed by their own notices.
11. Changes to this policy
We may update this policy to reflect changes in our practices or legal obligations. We will post the revised version here and update the effective date. Material changes will be communicated as required by law.
